Research Participant Consent Form

As a participant in a research study, it is required that you complete a consent form. It is recommended that you have a copy of this consent form for your records. However, it is understood that it may not be safe/secure for you to do so, therefore you can choose to not keep a copy and know to contact the principal investigator, Dina Idriss-Wheeler at didri040@uottawa.ca, if I would like an electronic copy sent or mailed.
